Immunization Record 4 DTAP (1 Dose on or after 4th Birthday) 4 POLIO (4th Dose on or after 4th Birthday and at least 6 months after previous dose given) 2 MMR 3 Hepatitis B 2 Varicella vaccine (chicken pox vaccine) Immunizations are mandated by the state. If your child is not properly immunized prior to the 1st day of school, they WILL NOT be allowed to start school. If missing immunizations, you will need a plan with scheduled dr. visits to complete the needed immunizations.

17 Medication Policy Medications- Parent/Guardian must bring in… NO BUS!
Bottle must have: name of the child date of prescription name of medication dosage amount specific time/circumstance of administration name of pharmacist/physician

Form filled out—in office or online Written order from doctor In labeled container School nurse will administer Adult must bring to office at the beginning of the day-- not on the bus

Designate all pick-up/drop off addresses on form. Stops M-F must be the same. Pick-up location may be different from Drop-off location. Early August, letter sent with teacher’s name and information on how to look up info in BusPlanner. Communicate in writing any changes for a bus pass. Bus passes are for emergency situations only. Any permanent changes must be made on the form found on the ELCO website (print out and return to transportation office). Any bus changes must be returned by July 15th to be effective the 1st day of school.

Attendance Absences Written excuse within 3 days of the absence 10 days total = “parent” excuse More than 10 days requires physician excuse School Attendance Improvement Plan—after 10 day letter has been sent Educational Trips Appropriate form must be completed 2 weeks prior to trip Maximum of 6 days of absences—counted toward 10 days

32 Sign-in & Sign-out Adults must come into the office to sign students in if they arrive at 9:00 AM or after Early dismissal- must sign students out Your name and relationship to the child Notes must be sent in with student in the morning telling of pick-up time and who will be getting the child

33 INFORMATION BE PROVIDED TO THE SCHOOL AT ALL TIMES!
VERY IMPORTANT THAT UP-TO-DATE INFORMATION BE PROVIDED TO THE SCHOOL AT ALL TIMES! Phone numbers, addresses, emergency contact information are very important for so many reasons!

34 Field Trip/Classroom Volunteer Requirements
Pennsylvania Clearances Required: PA Child Abuse Report PA Criminal Record Check Disclosure statement or FBI clearance if not a PA resident for past 10 years. Good for 5 years! Free for volunteering

VERY IMPORTANT FOR SCHOOL/LIFE SUCCESS!!! Phone calls/classroom visits s are best! 24 hours before if possible!

August—One or two weeks before school starts: Kindergarten Bus Ride Night & Meet the Teacher (letter will be sent home with date & time)
