If the Screening Assessment indicates that an individual does not currently require specialist mental health service response, information and/ or referral to other appropriate services such as GP is provided.

Individuals assessed as not currently requiring ongoing treatment and care by the mental health service will receive information and referral options as above.

Discharge Planning
Discharge plan includes:
Relapse Prevention Plan
Information to GP and/or other service providers
Advice on how to recontact mental health service again if necessary.
