6 Sample Students with Disabilities Syllabus Policy
X University is committed to providing equal opportunity for participation in all programs, services and activities.  Requests for accommodations by persons with disabilities may be made by contacting the Disability Support Services at or on the web at dss.univ.edu.  Once your eligibility for an accommodation has been determined, you will be issued an individual services accommodation form.  Please present this form to me at the start of the term and/or two weeks prior to the accommodation date (test, project, etc.).  Requests received after this date will be honored whenever possible. While this syllabus statement example is important and can help to meet the needs of students with disabilities, it is still focused too much on accommodations and does so in a reactive fashion.

7 Meeting the Needs of Diverse Learners: A UDL Syllabus Statement
As your instructor, I feel I have a responsibility to do everything within reason to actively support a wide range of learning styles and abilities.  As such, I have taken training and applied the principles of Universal Design for Learning to this course.  Feel free to discuss your progress in this course with me at any time.  In addition, if you require any accommodations, submit your verified individual services accommodation (VISA) form to me during the first two weeks of the course. This syllabus statement has been developed to be more proactive and supportive of ALL learners, while still addressing any need for accommodations.
