Make an appointment with CTA Supervisor for a site visit
Awarded Contractor is to draft the following forms and submit to Vale Construction Coordinator:
Formal Risk Assessment-PHR
CTA Contractor Work Permit (completed weekly)
Routed Map
Adverse Weather Guidelines
Road Use Authorization Form (Mon to Fri Dayshift)

Add haulage route to Landfill or PQ Ditch
Identify the hazards (road conditions, weather)
-will you need an escort?
-will you need extra lighting?
-will you need a sander/plow/grader?
-are the roads slippery?

Vale Construction Coordinator to arrange Risk Assessment Meeting.
-in addition to the PHR Attendee Requirements, the CTA Supervisor and worker Rep must be invited to review the PHR.

After the Risk Assessment Meeting:
The Vale Construction Coordinator will sign the CTA Contractor Work Permit.
The Vale Construction Coordinator & Vale CTA Supervisor will sign the Road Use Authorization Form.
Contractor Supervisor to review PHR with all workers accessing the CTA and sign the PHR Worker Sign Off Sheet.
