May include: Volume metrics (e.g. wRVU, new patient visits) Patient Satisfaction Quality At least 50% of the clinical goals must be based on wRVU target (adjusted for service allocation) In general, all goals have a: Threshold → Level to receive any portion of award Target → Level to receive full award wRVU goals also have a level of superior performance Level achieved triggers maximum allowable for Non-Clinical Goals Performance % Achieved % Award Below Expectation <15 of Benchmark 0% Threshold -15% to -6% of Benchmark 50% Target - 5% to +114% of Benchmark 100% Superior +115% of Benchmark 150%

2 Non - Clinical Incentive Goals
Examples based on Salary of $100,000, Clinical Allocation of 75%, and Non-Clinical of 25% Scenario #1 wRVU achieved is 82% of Benchmark, access: 0% Clinical Award ($0) Maximum 50% for Non-Clinical Award (max of $2,500) Scenario #2 wRVU achieved is 97% of Benchmark, access: 100% Maximum Clinical Award ($15,000) Maximum 100% for Non-Clinical Award (max of $5,000) Scenario #3 wRVU achieved is 118% of Benchmark, access: 150% Maximum Clinical Award ($22,500) Maximum 150% for Non-Clinical Award (max of $7,500) 1
