Plan 1)Install hub area in central pharmacy with assistance from the Scottish Government 2)Install spoke and hub IT solution using barcode scanning accuracy checking 3)Engage with NHS Boards, GP Practices and Prescribing Support Pharmacists to identify suitable patients for CMS serial Rx’s 4)Hub significant volume of Rx’s through central pharmacy 5) Monitor patient centred service provision at the spoke pharmacies Do Initial efforts to move patients onto serial Rx’s proved challenging depending on the agreed model of implementation with local stakeholders. NHS Forth Valley implemented a practice led model which has led to a large initial increase in serial prescribing. Community Pharmacy led model in GG+C has led to sporadic increase in serial RX’s. Fully functioning hub installed with wholesale accounts opened. Skilled hub team of 1 Pharmacist (0.4 WTE), 1 Accuracy Checking Technician (0.2 WTE) and 1 NVQ Level 2 Dispensing Assistant (0.1) allocated to the hub.
