5 Assisted Living and Skilled Nursing Facilities
Good Shepherd currently provides rehabilitation services to the residents of nine assisted living facilities and one skilled nursing facility. We expect to have formal agreements for two more assisted living facilities within the next month. We are proud to bring to these facilities the model of rehabilitation care and staff expertise that Good Shepherd is well known for. We believe that the elderly deserve the best life possible. Good Shepherd works with senior living communities to develop customized, proactive therapies that result in stronger, healthier residents who are at less risk for falls and subsequent injuries. Traditionally, these facilities do not receive the level of rehabilitation expertise that Good Shepherd provides. As a result, we have been very well received and are being asked regularly to expand our presence in these facilities.

6 Assisted Living and Skilled Nursing Facilities
Good Shepherd provides rehabilitation services to meet the specific needs of the residents in these facilities. Program offerings, among many, include the following: Fall risk assessment and reduction program. Dementia program – including cognitive rehabilitation and functional restoration and safety programs. Parkinson’s disease rehabilitation and wellness programming. Swallowing / feeding programs. Stay Strong Program – program that keeps residents fit, strong, and healthy in their residence.

7 (IHOP) In-Home Outpatient Therapy
IHOP is outpatient therapy where the therapist provides physical, occupational, and / or speech therapy in the patient’s home. This service is designed to reach patients who are unable to take part in traditional outpatient therapy. The goal of Good Shepherd’s In-Home Therapy is to help patients maintain a healthy lifestyle and to help them develop the necessary skills so that they can remain living in their home. Good Shepherd’s IHOP therapists also provide therapy services to patients living in residences in senior communities, independent living apartments and assisted living facilities.

8 (IHOP) In-Home Outpatient Therapy
Good Shepherd’s IHOP services provided in the home are reimbursable by Medicare as long as the patient is not receiving formal home health care services. As the patient improves, the patient may elect to transfer to one of Good Shepherd’s outpatient facilities for their therapy to take advantage of the state of the art rehabilitation equipment located there.
