3. Recovery Package & Stratified Pathway Recommendations:
‘Cancer Taskforce is working with Macmillan to roll out the ‘Recovery Package’… a set of actions that ensure the individual needs of all people going through cancer treatment are met by tailored support and services’
‘We know that risk stratified pathways …..can not only improve care for patients after treatment, but create more efficient pathways that can reduce unnecessary hospital appointments’

4. By March 2017, NHS England, through Cancer Alliances, will drive the spread of risk-stratified pathways, including supported self-management, for breast cancer, including assessing the role of commissioning incentives to encourage implementation
In subsequent years we will focus energy of rolling out this approach to other cancer sites

5. SWAG Cancer Alliance Living With & Beyond Cancer (LWBC) working group
Aiming to deliver Taskforce recommendations on a local level
Sub-groups focussing on content and quality of Recovery Package delivery across Alliance (HNA, TS and H&WB events)
Stratified Pathway review – aim to standardise approaches to follow-up across the Alliance

6. Review of Current Pathways
Call for examples of current pathways including evidence of any stratification across both SWAG and Peninsula Cancer Alliances
Review of pathways to improve follow-up care for patients – implementation of the Recovery Package
New ways of working especially for low risk, stable disease patients
Free up outpatient capacity for NP/ complex patients

7. Head & Neck Pathways Thyroid cancer pathways – review of previous work
Identify other groups of patients where follow-up changes are possible
Use of surveillance testing
Nurse-led, telephone follow-up, supported self-management with Recovery Package implementation

8. Treatment Summary Templates
Focus group working to produce best practice TS templates for each cancer site
Macmillan GP part of group reviewing content
Will help teams get started with TS
Will help with quality surveillance
Standardise information given to GP’s i.e. treatment effects; alert symptoms; GP actions; secondary care management

9. Request?
Clinical expertise to help to develop follow-up stratified pathways and to work on developing Treatment Summary templates
AIM: To develop SWAG Head & Neck stratified follow-up pathway guidelines which incorporates how the Recovery Package works in practice
