Drawing/Graphical Representation of a Heart, Viewed From the Lateral Side A quadripolar lead is wedged in a distal cardiac vein. The distal electrode (1) is positioned close to an area of myocardial infarction and an area of phrenic nerve stimulation, whereas 2, 3, and 4 are positioned near the optimal pacing site. The latter is possibly defined by electroanatomical mapping or speckle tracking echocardiography. IVC = inferior vena cava; LV = left ventricle; PNS = phrenic nerve stimulation; RV = right ventricle.
