Behavioral Health Convene a community collaborative to develop a comprehensive continuum of care for Behavioral Health, based on a current needs assessment for Lee County. The collaborative should identify a champion and incorporate successful models for both leadership structure and service. (Key elements may include inpatient beds, access to assessment, availability of medications, case management, supportive/affordable housing, staffing, criminal justice partners/resources, preventative care, and unique needs of adults , adolescents and children.)

Assembled a community Mental Health and Addiction Coalition which chaired by Bob Janes, Jim Nathan, and Brian Lucas Developed a strategic plan for the coalition Purpose: Improve the lives of individuals with mental health and addictions needs Mission: Define workable solutions to end the mental health crisis in our community

Goals: Implement a mental health and addictions delivery model to meet the existing and future needs of the consumers (Carol Conway) Obtain and responsibly allocate funding adequate to meet the needs (Richard Riley) Develop a community leadership group to implement the strategic plan (Bob Janes, Brian Lucas, Jim Nathan) Create awareness of mental disorders as a serious health issue (Lance McKinney)

Status Report: Key Accomplishments
Created a task force for each goal with community ownership Implemented Triage Center Converted 15 beds at Lee Memorial Hospital to a Geriatric Psychiatric program Launching the development of The Pavilion at HealthPark Expanded Services Key Accomplishments Triage Center - $1 million state grant for law enforcement diversion, matched with $2 million local funds over 3 years. Provides intake, medical assessment, mental health and substance abuse screenings plus a low demand shelter on the grounds of Lee Mental Health, Ruth Cooper Center. Partners are LMHS, Lee County Human Services Department, The Salvation Army, Lee Mental Health, SWFAS, all Lee County area law enforcement, United Way. Intake from 4 - midnight only. Referrals from law enforcement in lieu of arrest. Referrals from LMHS EDs after medical screenings of eligible clients. Over 30% of clients in the first 4 months accepted referrals to some type of treatment or support. Family Health Centers accepts referrals for follow-up medical care to establish a medical home. LMHS Geriatric Psychiatric beds - 15 beds at LMH are being converted to a geriatric psychiatric program, under management contract with The Pavilion at Lee Memorial, formerly Reliant Healthcare. First step toward permanent psychiatric care hospital beds in Lee County. Renovation currently underway in the Fall Open in January The management team of The Pavilion will use this immediate availability to establish a significant presence in Lee County, to recruit and train staff, to develop working relationships with physicians, patients and their families, nursing homes, and psychiatrists. This will build their capacity and help for a smooth transition to their anticipated free-standing psychiatric hospital. The Pavilion at HealthPark - The newly formed private company formed by executives from Reliant HealthCare and local investors from Collier County to build a 76-bed psychiatric hospital for Lee County in response to the shortage of psychiatric care in Lee County. The Pavilion has filed for its Certificate of Need with the State of Florida. Notice is expected by mid-December Construction plans call for completion and occupancy in late fall 2010 at a site in HealthPark. Plans are for an adult care facility (over age 18). Plans include recognition as a Baker Act Receiving Facility. Expanded Services: Encouraged and helped Jennings Behavioral Health come to our community, expanded MH and addiction services in the Cape through the Cape Coral United Way House (Cape Coral Foundation grant to Lee Mental Health for outpatient services) partnered with Hope House, (clubhouse setting concentrating on work placement for the Mentally Ill) at the East Ft. Myers United Way House, welcomed a new provider (Psy Care) whose specialty is working with adjudicated youth (they contract with the Marine Institute), Children’s Advocacy Center, and ACT expanded licensed mental health services internally because of the demand, and that Lee Mental Health asked and received approval by their United Way Allocations Team to reallocate funding so they could expand outpatient services.

Fragmented delivery of services and silos Restructure of DCF leaving no local presence Allocation and reallocation of funds to support the coalition goals Lack of doctors Merging of medical and recovery model of psychiatric care Community understanding and treating mental illness as a legitimate medical condition Ongoing community volunteer leadership to drive strategy implementation Expanding Triage Center to a full 24/7 operation CON approval of The Pavilion at HealthPark Barriers/Challenges Triage Center - Expansion to a full 24-7 operation. The grant will last for 3 years and replacement funding plus expansion to full service is needed. Will there be enough savings from law enforcement diversion to pursue reallocation of criminal justice funds to Triage? No law enforcement partners are funders now. The Pavilion at HealthPark - Pending approval of CON. Recruitment of psychiatric staff including psychiatrists.

Waiting on Medicaid Provider number for reimbursement for Jennings Behavioral Health Critical Success Factors Triage Center - The structure of a Policy Board and a Management Council that meet monthly to keep the partners and funders actively engaged in this collaboration to ensure smooth operation, monitor results, and seek long term treatment success for clients. LMHS Geriatric Psychiatric beds/The Pavilion at Health/Park - The combined strengths of the new company, The Pavillion, bring clinical competency, management experience, and new funding sources to Lee County for psychiatric care. Jennings Behavioral Health - Still waiting on Medicaid Provider number for reimbursement.

Review and define the best practices for mental health delivery which will be driven by evidence-based psychiatric practices incorporating medical and recovery models Develop and implement a comprehensive communication/marketing plan to build the desired level of awareness in our community Complete and implement other details of the strategic plan Expand leadership structure

Triage Center - Continue operational improvements, data collection and evaluation. Seek long term funding solutions. LMHS Geriatric Psychiatric beds/The Pavilion - Complete construction and open in 2009. Jennings Behavioral Health - Continued expansion from current 35 patients, 2 psychiatrists, and 4 therapist in O/P treatments. Next Steps Triage Center - Continue operational improvements, data collection and evaluation. Seek long term funding solutions. LMHS Geriatric Psychiatric beds/The Pavilion - Complete construction and open in Successful operation at LMH for approximately 2 more years. Plan phase out to The Pavilion when it opens. Jennings Behavioral Health - Continued expansion from current 35 patients, 2 psychiatrists, and 4 therapist in O/P treatments. Begin the trial program for the partial hospitalization program for both adults and youth. Continue pursuit of Medicaid Provider number. Continue outreach to other providers and patients.
