FIGURE Left common carotid artery (LCCA) angiogram in a 78-year-old man demonstrating significant carotid tortuosity with a 90° angulated ostium of the left internal carotid artery and critical stenosis distal to the bifurcation. Such carotid tortuosity poses significant challenges to the endovascular proceduralist and may contribute to distal embolization during carotid angioplasty and stenting and positioning of a distal endovascular protection device.
