The CREIGHTON MODEL FertilityCare Practitioner Program
Consists of 2 Education Phases and 2 Supervised Practicum, Site Visit and exam.
The Education Phases will be conducted at the Family Resources Center, 415 N.E. Monroe Ave, Peoria, IL 61603
The Supervised Practica are conducted in the students home location.
