1 Adding/ Editing New Immunizations
Welcome to the Adding/ Editing New Immunizations lesson for the North Carolina Immunization Registry.

2 Once you have located your client’s record, to document new immunizations begin by Clicking on History/Recommend. Once you click on Manage Client from the home page and search for your client’s record, you will be ready to administer immunizations. Click History/ Recommend.

3 The NCIR makes vaccine recommendations based on 4 criteria

4 By clicking New Immunization Entry By clicking Add Selected
There are 2 ways to enter a new immunization from inventory. By clicking New Immunization Entry Both options will take you to the same entry screen, the only difference is that Add Selected makes the green check marks for you and New Immunization Entry does not (you select the vaccines manually) By clicking Add Selected There are 2 ways to enter a live immunization from inventory. 5 When you’re finished, click OK
Choose the Site under Organization Site Make sure the vaccine is selected Choose your Ordering Authority Choose who administered the shot Choose the date the shot was administered Either by typing it in or clicking on the calendar When you click on the calendar it will pop up and default to the current day. Click OK Choose the organization under Organization Site Choose your Ordering Authority Choose who administered the shot Choose the date the shot was administered either by typing in the date or clicking on the calendar. (Note you can leave the date administered box empty and it will default to the current date) When you click on the calendar it will pop up and default to the current day. Just click ok. When you’re finished click OK. The Ordering Authority is the doctor that ordered the shot. Can be a MD, DO, NP, PA

6 You must first verify the patient’s Eligibility by selecting from the drop down box. You will not be able to progress past this screen without completing this step. The choices in the drop down box are the same choices that were available on the VAL forms. Click OK when finished Choose your Trade Name- Lot Numbers by clicking on the drop down arrow. **State supplied vaccine is in blue and Private supply vaccine is in black Choose your Body Site the same way- click on the drop down box and choose your site. The first thing you should do on this screen is to verify the Eligibility by selecting from the drop down box. You will not be able to progress past this screen without completing this step. The choices in the drop down box are the same choices that were available on the VAL forms. Choose your Trade Name- Lot Numbers by clicking on the drop down arrow. When you’ve selected all the information then you can click OK. This is going to redirect you back to the History/Recommend screen. As you choose the Trade Name and Lot Number the most current date for the Vaccine Information Statement will appear for each individual vaccine at the bottom of the screen. If you have chosen a vaccine incorrectly and need to remove it, simply click on the box next to the vaccine until the green check appears and this will remove that vaccine when you click OK. If you have not entered and/or saved the Eligibility status you will not be able to progress past this screen. Please pay special attention to the lot numbers that you choose to make sure they are the correct one. Notice that as you choose vaccines, the VIS Publication Date will display at the bottom with the most current date. You can choose the date of the actual copy that the patient received by clicking on the drop down box.

8 Eligibility as reported by Responsible Person
From the Edit screen you can change the following information for a live vaccine that your facility has administered: Date Provided Ordering Authority Administered By Body Site Administered Route Eligibility as reported by Responsible Person You are also able to add Reactions to IMMUNIZATION on this page by selecting the corresponding reaction at the bottom of the screen.
