3. Enter information in the required fields Next, click the Submit button
Remember that your Facility Number and FIN are printed on the mail out that you received from DSHS in September and/or December 2010
Next, click the Submit button

4. Next, click the Continue button
Write down your User ID and Password. You will use the same User ID and Password to login to the system next year.

5. Enter the User ID and Password that you wrote down
