DeCA Form 10-21 You Can not Claim Compensation without it.
Get a control number from the vendor Describe in detail the problem Include Pictures when possible Proper completions of DeCA From 1021 is essential for accountability, and quality assurance. ACCOUNTABILITY-Is the product received according to contract specification. If not call and request a compensation from the vendor. Forward this form to the Region meat specialist and Fort lee contracting. Obtain a vendor control number from the supplier for tracking purposes. QUALITY- Quality of product and packaging to included crushed case, and blown package should be photographed and forwarded. Report any incident which not conforming to contract specification.

Date and Time of Event Amount of Compensation Fax copies to HQ’s – (804) (DSN 687) Region –Reeves Menhennett at DSN DECA Form 10.21: Try if at all possible to submit your DECA Form Non Conformance Report within 24 hour of delivery. This is particularly important if is a transportation issue. Transportation is normally provided by a sub-contractor, and if the prime-vendor is to recoup losses from the delivery company timely submission is very important. If product is non conforming Non Conformance Report must be submitted, to Sheila Norfus, Mo Estrada, Lonnie Drumin (Beef)-James Shults (Pork)-Judge Mays (Name brand) and RTC contract holder. The contractor, Resale Contracting Division, and Region Meat Specialist must be notified of the amount of compensation you requested. Along with the contractor control number. Non Conformance Report must be have complete contractor identification, contractor POC, and clear discrepancies remarks. “If you find FOUR cases damaged on a pallet, claim only for the four cases not the whole pallet. Also claim only for the product which is damaged. CONTRACT: Go to Commissaries. COM, (click on) Business with DeCA, (Click on) Resale/Grocery Products and services. No changes are authorized at the store or Region level.

Quarterly evaluation/walk through by Store Manager will identify 90% of all discrepancies. Bi-monthly spot checks of two or three items will ensure compliance. SUBJECT: Clarification Regarding DeCA Meat I.G. Checklist Item, 9-MAN-PS This guidance will apply to all commissaries involved in the production and fat analysis testing of fresh ground meats and will remain in effect until rescinded, suspended or replaced by a DeCA directive. The noted Meat I.G. Checklist item currently reads as follows; 9. MAN-PS Q - Is ground beef processed daily, and has a Fat Analysis Test been conducted on each variety processed from table trim and the findings recorded on DeCAF 40-8, Fat Analysis Record for Ground Beef, and were nonconforming products properly reduced in price for quick sale (except Europe)? A Fat Analysis Test will be performed daily on each variety of ground beef processed. Nonconforming products will be appropriately reduced in price for quick sale. (DeCAD 40-3, paras 13-6, (C5) 14-1h and i) Ground beef is not mandated to be processed daily. Every effort shall be made to provide the freshest ground beef products as possible and sold within their specified shelf-life parameters. Fresh ground beef trimmings generated from table trim must be processed within the following time frames; Seven-day stores shall grind trimmings within 48 hours from processing; 5 and 6-day stores within 72 hours of processing. Bulk Coarse Ground Beef shall be randomly tested weekly and documented on the DeCAF 40-8 on each ground beef item manufactured from the soul component of course ground beef. On the spot test and documentation shall be preformed at any time ground beef products manufactured from course ground beef appear to be non-conforming per the products stated lean to fat ratio. Should there be no grind requirement on a particular day, then annotate the DeCAF 40-8 with the acronym “DNP” to be interrupted as, “Did Not Process”. “DeCA wide price” 81% ground beef program 81% Ground Beef Provide a quality product for a everyday low price of $1.29. The some areas Prices have changed to make this percentage affordable for less, but normally this percentage is priced to the point that the cost makes our lower rank customers settle for the 75% lean purely for monetary reasons.
