Figure Integration of deceased donor-directed donation, donor–recipient incompatibility, and paired exchanges in kidney transplantation. In instances of incompatible deceased donor-directed kidney donations, the kidney is allocated to initiate an exchange. As a result, the previously incompatible designated recipient on the deceased-donor wait list receives a compatible live-donor organ. This process allows for those in the paired exchange to be transplanted, and results in no net loss of organs and likely no blood type O disadvantage.
