Present – Quick Card at the machine with experienced operator. Tryout – Person runs the machine with supervision. Follow up – Competency points are verified by supervisor and is signed by the trainee/trainer to complete the training. Steps for Retraining: Review SWI or Quick Card Verify Competency Points Complete Annual Training Requalification Form Competency Points: Pre-op / Parameter Checks © Drilling © Installing Bolt © Tramming Place to Place© Operating ATRS © The following safe work instructions is NOT INTENDED to replace hands on training requirements of the code of federal regulations. The intent is to provide an overview of known potential safety hazards associated with equipment operation. Hands on training with an experienced operator on each specific model must be conducted. The trainee can not proceed onto production until they are determined ready to proceed by the supervisor of the training. Before any operation or maintenance procedure is performed with or on this equipment, it is important that personnel have a complete understanding of the location and functions of all controls and indicators, as well as, the location and use of all emergency shutdown controls and procedures. Title: Bolter - Mobile (Walk Thru, Dual Boom, Single Boom, Track Bolters) ) SAFE WORK INSTRUCTIONS KEY POINTS SAFETY PRECAUTIONS Pre-ops: Check plug, receptacle and breaker for proper I.D. tag. Remove power at power source. Check cable from plug to strain reel clamp. Check bolter for required safety tools and supplies. For example slate bar of proper length and proper torque wrench. Check over all conditions of machine for cleanliness and obvious mechanical defects. Make sure section has been examined before putting the power on your machine. Check headlights. Make sure you read tag correctly. Tag and lock out. Keep cable out of roadway and away from other moving equipment, observe roof and ribs. When pulling cable off of reel, note condition of level wind cable, spool and strain clamp. Report all damaged splices, exposed wire or insulation and have repaired before use. Follow proper lifting and materials handling procedures when supplying bolter. Effective communications must be maintained at all times between the bolt man & bolt man helper during bolt machine operations. This includes ALL aspects from supplying of the machine to bolt installation. Control switch in off position. Check for oil leaks and oil level. Check condition of hydraulic hoses for damage and wear and replace if necessary. Use sounding device before moving. Make sure all lights are in good condition and securely fastened.

Check all control levers. Start pump motor. Check dead man switch (If applicable) – switch must be maintained in proper operating condition. If the machine is equipped with the Articulating feature be sure to check that lever is self-centering and functioning properly. Make sure the rail clamps, drop mast, and outriggers are raise and released, the drill mast is lowered, and parking brake is released. (Track Bolter) Tram bolter at a speed consistent with section or track conditions. Stop bolter under supported top. Tram bolter to first hole to be drilled. Bolt machine should not drift (Move) when positioned. If machine moves corrective action must be taken. Examine roof, roadway for debris, ribs and test for methane gas. Examination of roof and rib includes sound and vibration. Scale roof if necessary. Follow procedures in roof control plan. Make sure levers are centered. Make sure brakes are set (if applicable), make sure panic bar will shut off machine then reset if. Correct substandard conditions prior to operation of machine. If unable to correct notify your foreman immediately. Be sure all employees are in the clear. Sound warning bell or state “starting up”. Keep boom in center of entry and have helper to watch cable for clearance, do not run over cable. Note: The drill boom must be positioned straight ahead and sumped all the way back Be sure to stop pump motor, set safety jacks or ATRS and always work under supported roof. Raise canopies if applicable. According to your roof control plan, make sure temporary supports are clear of bolting area. Be sure to have clearance between curtain and bolter. If ATRS is used, follow applicable safety precautions in roof control plan. If installing rib ALWAYS MAINTAIN PROPER BODY positioning

Never stand or allow anyone to stand between the machine and rib. Use the opposite side controls or if needed turn the machine around for rib bolt installation. Double boomed bolters with outside controls follow specific procedures as outlined in the roof control plan. Drilling: After the machine has been correctly positioned for the first row of bolts, the ATRS is set against the roof if applicable. Set parking brake (if equipped). Set rail clamps. (Track Bolter) Lower the leveling jacks or outriggers. (Track Bolter) Prior to drilling examine the roof and ribs for loose materials. Check for methane. Examine drill steels for burrs and straightness. No person is to go inby unsupported roof at any time. Always follow the sites approved Roof Control Plan. Use a bar of suitable strength and length for scaling loose material. Machine will be unstable if rail clamps are not set before drilling. (Track Bolter) Always check for gas before energizing a machine on and at least every 20 minutes thereafter or more frequent if required in the approved Ventilation Plan or conditions warrant. Drill steels must be straight and free from damage or defects. Never mark steels by scoring. Position yourself so machine can be shut off immediately. Keep feet and legs from under bolter head..

Utilize proper body positioning and hands off drilling. Do not touch, hold onto, grab or reach for rotating drill tools (wrench, spanner, steel, auger, extension, drill steel or bolt) with hand while drilling or installing bolts. Ensure the hole is drilled to the proper depth Be aware of pinch points. Use caution when working with a suspended steel. Warning: Never place your hand or other body part on moving or pressurized components. Always use “HANDS OFF” DRILLING METHOD. Serious injury or death can occur. Watch for pinch points between the two (2) augers or drill steel. Always connect steels or augers and lower them to the drill head. Watch for pinch points between tray, hot steel and bits. Steels are to be placed entirely within the confines of the tray. DO NOT allow steels ends to extend up or over the edge of the tray. If drill steel becomes caught in hole, do not place hands or any part of body below auger. All actions need to be directed at removing the suspended steel safely. Be sure rotation is stopped. Keep feet in the clear of the unit. Ensure that all personnel in the area are aware of your intentions. Hearing protection is required during drilling.

Examine bolt wrenches for defects and compatibility with bolts before use. Utilize proper body positioning and hand placement. Check the torque pressure gauge to ensure that bolts are properly torqued within range of approved plan. When finished mining/bolting in a location or leaving the section ensure that unsupported roof or danger signs are installed. Check resin for condition and expiration date Utilize safe lifting techniques when handling materials. Do not use damaged or defective bolts/wrenches. Keep Hand away from pinch points. Wrenches must be manufactured. (No homemade wrenches). Do not hold wrench with hand, utilize a tool to guide bolt/wrench while in motion. If the bolt/wrench must be positioned by hand, all motion must be stopped and pressure released before placing hand in red zone. Store bolts and bolt wrenches in proper designed locations. Keep tray in an orderly condition. Watch your fingers when putting the bolt against the resin. Avoid pinch points. Verify bolt meets torque specifications in Roof Control Plan. Note: Do not place hands on bolts that are being raised. Unsupported roof/danger signs are required and must be reflective and visible. Do not use resin that is damaged or expired. Never ride on head of bolter.
