This presentation has been prepared for Sanjay Sundram to help meet the requirements of the Instructional Designer test. This is only a sample to illustrate some my technical skills and creative abilities. The proceeding slides will accompany and illustrate the objectives of this test presented in the Word document. 6 Learning Objectives Once the modules are complete, the participant will: recognize steps required by law to be performed in the hazard recognition process define hazards and recognize permission procedures to enter danger zones identify causes and prevention of hazardous situations in the workplace

7 Assessment - Short Answer Questions
This assessment will determine your ability to recall information after viewing the Lesson 3 Module. Please answer the following questions: What is the third step in the hazard recognition approach? What is a question to ask when identifying a hazard? What is the distance of the danger zone usually? What is one reason why people cause hazards? What is one way to reduce hazards related to tools and equipment? Who provides instructions to be followed for controlled substances? What is one cause of incidents at the workplace? What are two housekeeping tips?

8 Interactive Activity – Hazard Search
This activity will assess your ability to search and identify hazardous areas using an interactive diagram which can be displayed by the link below Once the diagram is displayed, place your mouse pointer over an area that you feel can be a hazard. Click the area and if is it correct, an “X” will appear and a description will also appear to the right of the diagram. If it is incorrect, a response will appear on the right of the diagram indicating it is not a hazard. To display this diagram, click on the words “Diagram Illustration Quiz”.

This case video will assess your ability to analyze a workplace incident and critically think how to resolve it. Watch this brief video and answer the following questions: What was used to help prevent the accident? What else could have been done to prevent the accident?
