4. Goals of the Opioid Review and MAT Clinic
ECHO
Aid providers in the appropriate management of narcotic prescribing.
Give providers the ability to identity patients that are not appropriate for opioids, through things such as chart reviews.
 Be able to identify comorbidities that put patients at higher risk of death.
Collect data for the state that will demonstrate that we can improve prescribing practices and decrease the number of pills being prescribed.
 Increase the number of buprenorphine providers in rural Minnesota.
Educate physicians on the CDC and state guidelines.

5. Data We Want to Collect
How many patients have had their controlled substances discontinued (this you will begin tracking as the program progresses).
Why were they discontinued, how many pills per month, per year
How many patients have received 5 or more opioid prescriptions in the last 12 rolling months- this can be collected through the EMR
Number of patients on Suboxone (we understand some may not have any Suboxone patients at this time)
Number of opioid overdoses in your county, and if possible, the Narcan use in your ambulance services. (If you are able to obtain this data)

6. Data We Want to Collect
A list of patients on controlled substances. We only need to know the numbers for your clinic.
Contact the main pharmacy in town and obtain data on the number of prescriptions of controlled substance opioid pills being dispensed per month, as a baseline piece of information.
 How many patients have controlled substance agreements on file.
