Background This investment is a result of a successful bid made to the Greater Manchester Transformation Fund as part of Bolton Locality Plan The Transformation fund aims to support savings and efficiencies in health and social care provision and the development of new models of care. Asset based approach Developing skills and confidence to stay healthy Asset based approach – we are not just patients or service users we are also a resource also most of the time we are looking after ourselves and others

Encouraging people to reduce their alcohol consumption Supporting people to have a healthier lifestyle Helping older people reduce their risk of falling at home or outside Supporting people to feel less isolated Supporting people to improve their mental wellbeing Enabling people to get involved in their local communities Improving people’s access to community groups and activities

Encouraging people to be more physically Active & Supporting people to improve their mental wellbeing Wave Adventure’s Funding to develop their Young Mentors Group who will be taking on lead roles, mentoring others and team building; developing their skills and confidence along the way

Special Needs Under Fives Support
Funding to support, manage and train their volunteers who enable children with special needs to join in with mainstream pre-school settings. Lynda and Kevin are part of part of a 50 strong volunteer team giving one to one support to children aged 2-5 who have additional needs.

Encouraging people to be more Physically Active & Reducing Social Isolation
Motiv8 2 Activ8 A female only group based in the Crompton area of Bolton, funded to organize 3 hours of physical activity for girls and women aged from 8+ Providing support, encouragement and fun!

Encouraging people to be more physically Active
Bolton Rugby FC Help the young people to get fitter and more active by introducing more of them to Rugby Union. The project has been funded to provide after school introductions to Rugby Union in 6 schools a year. Sessions will be will be none contact and emphasize ball handling, fun and fitness. With everyone having the opportunity to represent their school in local tournaments.
