Overview of Individual patient care planning summary reports
Health Board/Trust users will be able to access, download and print off a Care Planning summary report for individual patients as an added bonus feature when using the Epilepsy12 data platform. N.B. All data displayed within this guide is fictional.
The report provides a summary of ongoing aspects of care planning which have been recorded in the Care Planning section of the respective patient’s clinical audit forms. It references details of aspects of care planning that have been discussed with the patient and parents/carers, when those discussions took place and which members of the clinical team provided the information.

What is a patient Individual Care planning report
Step 1
There are two ways to access a patient’s Care Planning report. Option 1 is to select the ‘Reports’ tab on the Home Page.

Step 2
Using the drop down options under By case ID, select the name of the patient and click either the computer icon to view the Care Planning report on screen or click on the PDF icon to download a copy of the report.

Step 3
The Care Planning report will display the data that has been entered for the chosen patient within their respective clinical audit forms. The data is within the report is added cumulatively.

The Care Planning Summary report can also be accessed directly from a patient’s clinical audit form (see user guide - accessing the clinical audit form). Click the ‘Care Planning Report’ button to view the report

Step 5
The Care Planning report will display the data entered for your patient within the clinical audit form. The report can also be downloaded and printed.
