In-School Sports Physicals Date : Friday, April 20st 2018 : Well-Beings will be offering sports physicals at Liberty Middle School again this year on April 20th Payment : $30 per child. Please send in a check or cash with your child to school on April 20th. Checks should be made out to Well-Beings, LLC. Paperwork : The GHSA Sports Physical Form and other required forms You must also fill out the “Consent to Treat Form” in order for your student to be able to receive a sports physical at school.
