Fig 1 A proposed mechanism by which exposure to air pollutants affects the development of AD. For example, transplacental exposure to ETS during pregnancy causes epigenetic changes, such as upregulation of microRNA-223 or hypomethylation of the TSLP gene CGI. These changes reduce the number of regulatory T cells or increase the expression of TSLP protein, leading to TH2 polarization. Postnatally, the skin is directly contacted by a variety of air pollutants, which produce ROS and reactive nitrogen species. This oxidative stress leads to damage to proteins and lipids in the epidermis, subsequently resulting in skin barrier dysfunction. Prior immunologic dysregulation or TH2 polarization induces skin barrier dysfunction and vice versa. Damage from air pollution could occur in a normal fetus, an infant, or a young child who is at the developmental stage or in a subject with genetic predisposition. Another route of exposure to air pollutants might be through inhalation or ingestion. Animal experiments and in vitro studies suggest that release of nonallergic proinflammatory cytokines, induction of itching and subsequent scratching, or an increase in the expression of IL-4 possibly contributes to the development of AD. Although transplacental exposure to air pollutants might directly impair the skin barrier in a fetus, there has been no evidence thus far.
