District of Columbia Department of Human Services Process Overview – Client Identification to Lease-Up Identify clients from VIS data Enter clients info into QuickBase Complete applications for eligible clients Review applications and submit to DCHA Review application s - DCHA Clients not currently housed in independent apartments but expected to transition to independent apartments Clients already housed in independent apartments and paid for with local funds Enter client demographic s and provider name/agency into the OCTO QuickBase system Identified eligible clients &complete HCVP applications with assistance of providers/agencie s. Note any special needs into the OCTO QuickBase to better match clients-units. DDS, DMH and DCOA review client applications, ensure that they are complete and provide the originals to DCHA. Enter date of application submission into OCTO QuickBase DCHA reviews applications and provides status update on application (approved, rejected & reason, or returned & reason) within 2 weeks of application submission If the unit is close to rent reasonable standards, negotiate with landlord Clients complete applications that are missing information. Agency staff or client providers serve as additional support Enter client demographic s, provider name/agenc y and unit info (location, size, rent, utilities) in the OCTO QuickBase system DCHA to enter rent reasonable data for unit specifications Rent reasonable unit Not rent reasonable unit Determine if housed clients can be converted to HCVP voucher Match individual s to units Lease-up DDS, DMH and DCOA match individuals to a rent reasonable unit that have passed inspection (or individual s current unit that is rent reasonable and has passed inspection) Schedule lease up with client and landlord at DCHA. Reflect accurate info in OCTO QuickBas e
