Ideal Future State JSSA embodies what it means to be Jewish and the Jewish community recognizes JSSA’s importance to how we live our values Goals 2021 Major Initiatives JSSA offers programs and services that integrate Jewish traditions and values into the delivery and approach to care Enhance services and staff education to reflect an explicit Jewish-informed approach to care across all services and programs JSSA achieves enhanced awareness and recognition in the region for its programs and services JSSA is recognized by the Jewish community as a meaningful place where the Jewish value of Tikkun Olam is put into action Implement comprehensive regional branding initiative to enhance market awareness of JSSA services across all communities Enhance outreach services throughout regional community

Ideal Future State JSSA provides a range of services that are financially viable, aligned with the mission, and inspire community support Goals 2021 Major Initiatives Multiple JSSA programs will support financial viability JSSA steadily achieves service and revenue growth JSSA will adjust its mix of services to support the mission and future sustainability through a thoughtful review (i.e., MacMillan) Implement innovative revenue tactics for current programs including partnership programs with payers JSSA has a robust philanthropic platform inspiring giving across generations to support innovation Implement new development programs to engage the next generation of JSSA supporters Enhance longstanding relationships to increase contributions to JSSA JSSA has an engaged and high-performing governance structure and processes Utilize governance best practices in structure and operations Implement individual, committee, and overall board accountabilities to enhance engagement

Organizational Integration Goals and Major Initiatives for Discussion
Ideal Future State JSSA has robust systems and processes to support provision of holistic and integrated care across the agency; JSSA’s care approach is outcomes-focused and data-driven Goals 2021 Major Initiatives JSSA programs and services are accountable for seamless, timely care and exceptional service delivery Develop and retain an engaged, highly skilled workforce Achieve standardized care pathways, models, and best practices1 across all services to enhance care coordination to meet client needs Define and implement holistic care, integrated across JSSA programs, to address the unique needs of individuals and families2 JSSA has a robust organizational infrastructure to support the future growth, optimal service coordination, and management of the agency Optimize organizational structure3 to support integrated care Offer one centralized and coordinated access point for all JSSA programs and services (i.e., Intake) Implement an agency-wide information technology system to allow for ease of communication and information transfer among programs Execute a standardized referral process across the agency and a consistent approach to staff communication and education JSSA demonstrates value through achievement of desired outcomes across all major programs Track and communicate outcome measures for all major JSSA programs (e.g., quality and service benchmarks, waitlist, client- driven needs)

Care Continuum Goals and Major Initiatives for Discussion
Ideal Future State JSSA partners with the community to operationalize a continuum of care to meet individual/community needs (emotional, physical, social, spiritual and family well-being) rather than only providing stand-alone services Goals 2021 Major Initiatives JSSA partners to provide the community with access to a full continuum of health and social services Develop and sustain relationships with 3+ local health systems and other agency/institutional providers (e.g., beginning with pilot programs to test services, outreach, and education) Create innovative integrated care models with systems/providers Execute standardized referral processes for programs and services not provided within the agency Identify and implement a viable approach for hospice inpatient services (e.g., partnership) JSSA enhances its community presence, serving additional populations1 than it does today Identify geographic and/or demographic areas and targeted growth opportunities for selected products/services2 Pilot and test novel approaches to deliver programs in targeted growth areas (e.g., through partnership with another agency, targeted philanthropic program) Populations may include geographic populations (e.g., cities/towns), demographic populations, or other.
