However recently accident totals are on the rise
2017: 24 to date with 8 being FSI –
2017 Part 135: 5 and 3

3. Factors also include improper installation (2), and one each of bird strike, carburetor ice, cg out of limits, hard landing, improper maintenance, mismanaged controls, over gross weight, stolen aircraft, and system failure.
2016 Factors are: Pilot induced error (10), lost power (2), weather (1), and component failure (1).

But there is a recent trend since 2014 of FSI Events increasing with a commensurate increase in fatalities and serious injuries

However, in 2016, accidents in the cruise phase of flight surpassed those in the landing phase of flight by a count of 8 to 3.
