Changes that can be made: HEALTH INSURANCE Enroll/Drop Health Coverage Add/Drop Dependent Health Coverage If you have no changes to make nothing needs to be done. FLEXIBLE SPENDING ACCOUNT (FSA) Enroll in FSA for 2018 If you wish to participate in FSA for 2018 you MUST enroll. Your elections will not automatically carry over. New Life Insurance Provider
*New Life Insurance Provider for 2018* Same as Lincoln, everything in Cigna will mirror what we had in Lincoln Group Life Insurance, AD&D, Long Term Disability company paid Group Life Insurance- 1.5 your annual salary AD&D- 1.5 your annual salary (in addition to standard life) Long Term Disability- pays 60% of annual salary after being disabled for 90 days

Please confirm your dependents names when reviewing your benefits. Failure to update incorrect information could result in a personal fine from the IRS. Please review all listed dependents in Oracle to make sure their name and SSN match what is listed on their Social Security Card.

FSA is a Pre-tax savings account that can be used to pay for medical expenses. Medical FSA Maximum contribution is $2,650 for 2018. Convenient Debit card for medical FSA program. Dependent Flexible Spending
Pre-tax savings account that can be used to pay for dependent care (day care) expenses. Dependent Care FSA Maximum contribution is $5,000 for 2018. Flexible Spending – Parking
Pre-tax savings account that can be used to pay for parking expenses. Parking FSA Maximum contribution is $255 for 2018. WWT covers $150 of this cost, should you choose to enroll. Flexible Spending Basics
Our FSA Plan is administered by UMR. Enrollment is done via Oracle Self Service, December 1st – 15th. The Plan year runs from January 1st – December 31st. Grace Period runs until March 15th. This means you have an extra 2 ½ months to use your FSA dollars. After March 15th, any money remaining in the account is forfeited. If you still have funds available, you can visit the FSA Store on Perk Path’s website. This site is full of FSA eligible items, click HERE to access the site! FSA is deducted on 24 out of the 26 pay periods. Contributions are pre-taxed, ultimately saving you money. Convenient Debit card option for medical FSA. If you already have a debit card, it will be loaded with the amount you elect on January 1st. Always save your receipts. You may have to submit them to UMR for substantiation. * If you take prescriptions, buy contacts or glasses, reach your deductible, or have copays throughout the year, you should be using this benefit. It’s a convenient and easy way to reduce your taxable income and save money!

Retirement Plan - Merrill Lynch
401(k) Plan Retirement Plan - Merrill Lynch Eligible after 3 months of service. Employees can rollover immediately. Employees can enroll 1st day of the following month. Set aside from 1% to 90% of your eligible compensation. WWT has discretionary match of 100% of your contributions up to 6% of your eligible compensation.
