Assessment tool had to be translated into Spanish
Time was an issue, not all assessments could be completed during medical visit
New tool was utilized and completed with 5 clients from 3 clinics total of 15 clients

Clients took notice of new tool, asked more questions and engaged more during assessment
5 out of the 15 clients were referred to further services (1 food pantry) (1 to case management) (2 to Outpatient Mental Health)
We continue to utilize new assessment tool in all 3 clinics
We continue to ask for client input about new tool and will continue to make changes as needed
