1. Rapid Assessment Clinic for the Older Person Aim
Provide a one stop multidisciplinary rapid Assessment & treatment for frail elderly patients who are not urgent but need to be seen in a short time frame, or who are deteriorating without cause
To support GPs in managing frail older patients
To operate a step up/step down service from other pathways (REACT, GPSU,…..)
Criteria
Following GP assessment the patient requires early geriatrician input
Deterioration medical or functional deterioration E.g breathlessness, confusion, self neglect
Most likely palliative without a confirmed diagnosis
Have isolated cognitive problems, co existing with other geriatric syndromes
