1. Trailer Wheel Came Off While Moving
HSE LEARNING 
INCIDENT:
An articulated water tanker was travelling on a graded road towards the Nimr-Marmul blacktop road when it lost two of its right side rear wheels from the articulated tanker trailer. The wheels, after coming off continued on and crossed the blacktop road coming to rest 40 meters from the lorry. It could have caused a serious crash.
ROOT CAUSES: -
LEARNINGS:
Check if your fleet wheel bearings are in good condition at least every 10,000 km.
Key Learning:
Check if your wheel bearings are safe.
