For new referrals
 CYPS FACE risk profile is used for all young people with enhanced needs up to the age of 18
For young people with enhanced needs who remain in service with NTW post 18 at their next care co-ordination review (which would include a ward discharge review) the Adult FACE risk profile will be completed unless they are transitioning out of NTW services when clinical judgement will be used to determine when (or indeed if) a different FACE risk profile should be used.  
This will cause some additional work i.e. transferring information from the CYPS to the Adult Face risk profile ( but copy and paste is a wonderful thing) it will be consistent across services that support young people and support transitions  between services.
Existing young people in service
clinical judgement will be used to determine when (or indeed if) a different FACE risk profile should be used. This judgement should be informed by the principle of supporting transitions between NTW services.   
A Serious Incident investigation identified
That the young person who was being supported by EIP and CYPS had 2 risk assessments (Adult and CYPS FACE risk profiles).
There was no consistency across EIP teams when working with a service user under 18 in relation to the appropriate risk profile to be used to record assessment of risk
There was no consistency across CYPS when working with a service user over 18 in relation to the appropriate risk profile to be used to record assessment of risk
