9 M2 Tips Task: In order to achieve M2, you will need to discuss how practitioners and agencies involved in the care pathways work together to provide the care needed for you chosen physiological disorders. Format: Q and A: you will need to answer a set of questions answered in the context of each of your physiological disorders. If possible you could interview someone who is personally involved in a multiagency approach for your chosen physiological disorders – or your fictional case studies for care pathways.

10 D1 Tips In order to complete your D1 task, you will need to use the information gathered in your P4 and M2 and write an overall conclusion on the effectiveness of health and social care practitioners and agencies working together to deliver car pathways for one of your chosen physiological disorders. You will need to use your Q and A to tease out 3 overall strengths and 3 overall weakness of this combination of people working together to achieve the best possible care for the individual. You will need to come to an overall conclusion as to how effectively based on the strengths and weaknesses identified the pathway is working for your client, with reasons referring back to the practical issues identified.

12 Example – not to be used One strength of HSC practitioners and other agencies working together when delivering a care pathway for an individual with Alzheimer’s disease, such as a neurologist and general practitioner is that the neurologist through detailed investigations will be able to monitor the progression of the condition and any changes which occur. This can then be communicated to the general practitioner who will ensure that the patient is given the correct treatment/ medication in order to stull maintain a certain level of independence in their day to day life.

13 Example – not to be used One weakness of HSC practitioners and other agencies working together when delivering a care pathway for an individual with Alzheimer's and a care assistant providing day to day care could occur due to conflicting schedules. The conflicting schedules and responsibilities of the care assistant could make it difficult at times for them to be able to work cooperatively to ensure that the client is fully supported with their condition.
