1 Mandatory Open Enrollment
Hope In Home Care Mandatory Open Enrollment As an employee of Hope In Home Care, you are required to talk to a benefits counselor at NES . You must call to schedule an appointment, please call : to reserve your time with a counselor. Open Enrollment will be held : 11/10/14 – 11/26/14. We will be tracking participation on these calls and are requiring 100 % compliance of each employee. As outlined under the new healthcare law, Affordable Care Act (ACA), all individuals must have Minimum Essential Coverage (MEC) beginning January 1, 2014 or pay a tax penalty. In 2015, the tax penalty for not having healthcare coverage increases to the greater of 2% of adjusted household income or $325 per adult plus $ per child. Employees can prevent being taxed the “Individual Mandate” tax penalty by purchasing one of two (MEC Plans) offered by Hope In Home Care effective January 1, 2015. Hope In Home Care is pleased to announce effective January 1, 2015 that we will offer health plan options that satisfy the ACA individual mandate required under the new healthcare law. In addition to these health plans, we are also offering supplemental health options through Allstate Insurance. These plans supplement your health benefits to alleviate financial strain due to catastrophic claims. This allows you to retain your hard earned money by paying cash benefits to you in order to offset high copayments, deductibles and non covered services under the plan. The Allstate policies will pay you directly for an unforeseen illness, injury or accident. Minimum Essential Coverage (MEC)- Option 1 This MEC Plan covers 100% of the government’s listed Preventive and Wellness Benefits when you visit a network provider . Offered by Hope In Home Care, this coverage is made available to you to satisfy your individual mandate under the new ACA law. MEC provides first dollar coverage for preventive and wellness benefits with access to one of the largest national preferred provider organization networks (PPO) available. The plan comes with a medical ID Card that needs to be presented to your medical provider at your time of service. The MEC Plan is a very affordable option that covers the 63 preventive services at 100% that are required under the government list of Preventive and Wellness Benefits. To review the list of participating primary care physicians – please visit the Multiplan website below and click on the find a provider link For a complete plan summary and listed exclusions and non-covered services please ask your benefit counselor at NES to review plan provisions. Minimum Value Plan (MVP)- Option 2 Similar to the MEC Plan, the MVP Plan, will allow you to avoid the individual ACA penalty as well. In addition to the same coverage the MEC plan offers, The MVP Plan covers selected medical services such as physician visits, emergency room, lab & x-ray, high level imaging as well as prescription drugs. These plans offer low copayment services up to individual out of pocket limits up to $1,850 per calendar year. After this maximum is achieved, members would receive selected covered medical services at 100% for the remaining calendar year. Please ask your benefit’s counselor to review exclusions and limitations and also refer to official plan documents . Services such as Hospital Inpatient, ambulatory surgical services, outpatient surgery physician, maternity benefits, organ transplants and specialty drugs are not covered services under this plan. Below is a brief summary of covered services; however, please refer to the official plan documents for all covered services as well as exclusions. We will be tracking participation on these calls and are requiring 100 % compliance of each employee. *NEW Allstate Voluntary Supplemental Health products These plans are guarantee issue and require no health exams for this enrollment only. HOSPITAL INDEMNITY BENFIT Plan pays $1500 hospital admission benefit (no limit to number of admissions annually) Plan pays $1500 for Surgeries Plan pays daily hospital confinement benefit of $250 per day, for a duration up to 10 days Plan pays intensive care unit benefit of $250 per day, for a duration up to 10 days Employee, employee/spouse, employee/child(ren) and family coverage is available Benefits paid directly to employee, unless assigned to a provider ACCIDENT INSURANCE Depending of level of coverage employee selects – benefit can pay up to $750 for ER visits, $1,500 upon initial hospital confinement, $300 for ambulance service, $40,000 - $60,000 for accidental death and many other accident related benefits Plans can pay up to $75 for any doctor visit outside of a hospital 24 hour coverage Employee, employee/spouse, employee/child(ren) and family coverage available This benefit is fully portable CANCER INSURANCE Plan covers cancer, muscular dystrophy, multiple sclerosis and eighteen other specific diseases Plan pays $100 annually for specific wellness tests, such as Pap smear, chest x-ray, colonoscopy, prostate exam, etc. Individual and Family coverage available CRITICAL ILLNESS INSURANCE Employees can choose $10,000 or $20,000 benefit Plan pays a lump sum benefit when diagnosed with a heart attack, stroke, paralysis, major organ transplant, end stage renal failure, cancer, advance Parkinson’s or Alzheimer’s disease, benign brain tumor, coma, complete blindness or loss of hearing Employee, employee/spouse, employee/child(ren), and family coverage available This benefit is fully portable UNIVERSAL LIFE INSURANCE Permanent life insurance, that builds cash value Choose level of coverage to protect your family’s financial security Spouse and child(ren) coverage available This benefit is fully portable Important Benefit Highlights Satisfies Individual Mandate under the ACA.
