1. The Douglas County Mental Health Initiative
Changing the way Mental Health Services are delivered in Douglas County

2. The Douglas County Mental Health Initiative was formed in June 2014 in response to several tragic events in our community and, to address unmet mental health needs of county residents.
Today, the Initiative has 40 organizational members and continues to develop strategies to build an integrated mental health system for county residents including implementation of the Community Response Teams (CRT) and the Mental Health Navigator Program.

4. Community Response Teams
Beginning May 8, 2017:
1,686 encounters with 831 unique individuals
642 active 911 calls for service
1,044 follow-ups and referrals
111 field medical clearances by Fire/EMS

5. CRT Results From May 2017 – August 2018:
59% of 911 calls treated in place
15% of 911 calls resulted in direct mental health placement
246 Emergency Department & 107 Jail Diversions
852 Patrol Officers released back into service
165 Fire/Rescue employees & 85 fire vehicles released back into service
“Treated in Place” : Crisis evaluation, safety planning, resource brokering, de-escalation
“Direct Mental Health Placement”: AHN WIC, Bridge House, Children’s Hospital, AUMH WIC, Denver Springs, FRP, Highlands BH, Jefferson Hills, Porter

6. The Mental Health Navigator (MHN)
Provides short-term intensive navigation of the Mental Health System for individuals and families referred to the Office of the County Attorney (OCA) for possible in-patient and out-patient mental health and substance abuse involuntary treatment holds.
Strategic coordination and community care plans (case staffing) with representation from County Attorney, CRT, Human Services, Colorado Access, care providers, etc.
Fill gaps such as – coordinating first outpatient MH tx after hospital discharge, transport to tx, follow-up to ensure engagement with services/fit of services
Operate as part of the community system of support
Explore various resources
Provide referrals and resource direction
Client advocacy, networking and support to family members
Support clients in their accessing of appropriate clinical interventions and community support resources

7. The Mental Health Navigator

Supports clients in their accessing of appropriate clinical interventions and community support resources
Currently working with 10 adults – 7 carried over from the previous MHN who began work in January /7 the current MHN continues to work with intensively. 2/7 were connected to community based services and closed.

Supported by the Board of County Commissioners, County Management, local law enforcement and fire/EMS, mental health treatment providers and many other partners
Co-responder teams are unique in including fire/EMS, in-depth follow-up, and case management as critical to the CRT model

CRT Teams have 3 specific goals:
ED diversion for mental health crisis unless medically necessary
Jail diversion when mental health is the primary issue
Facilitate connection to appropriate mental health resources
Focus on the entire mental health service delivery system in our county - crisis and beyond

10. Julota Implementation – care coordination software system for CRT and partners
“No Fail” Case Management Network and Specialized Case Management
Additional Law Enforcement Partners for CRT to cover County and all municipalities
Integrated Mental Health System for Douglas County – County contract for system program and funding model
