2 Chronic kidney disease (CKD) is a growing problem in the United States.
The urgent need for supportive services regarding hemodialysis is apparent when looking at the volume of patients requiring this life-saving procedure in northern Michigan. These patients often lack information and treatment surrounding their disease. Our intent would be to create a resource center which would provide public information and facilitate services for these patients and their families. Chronic kidney disease (CKD) is a growing problem in the United States. In fact, according to the National Kidney Foundation, more than 26 million Americans have chronic kidney disease and most don’t know it. Additionally, the number of people diagnosed with kidney failure has doubled each decade for the last two decades and the incline is expected to continue The urgent need for supportive services regarding hemodialysis is apparent when looking at the volume of patients requiring this life-saving procedure in northern Michigan More than 400 end stage renal disease patients require services in northern Michigan alone. These patients often lack information about the treatment surrounding their disease. Our intent is to create a resource center which would provide public information and facilitate services for these patients and their families.

Northern Michigan Hemodialysis Resource Center is dedicated to providing supportive services to dialysis patients being treated in Northern Michigan. Our resource center promotes quality of life and superior care specific to individual needs. At the Northern Michigan hemodialysis Resource center we stand by our commitment to those faced with end stage renal disease

4 Philosophy We believe that all patients have a right to receive quality education that can be utilized in making informed decisions with regard to end stage renal disease treatment, including dialysis. We believe that all patients have the right to access comprehensive and collaborative resources involving end stage renal disease, and that they should be able to receive this information from one center/source.  We believe that all patients have the right to the highest quality of care and professionalism. We believe in embracing cultural diversity.  We believe in using evidence-based practice as a means of quality assurance.  We believe the services provided by the Northern Michigan Hemodialysis Resource Center should be available to all in need, regardless of ability to pay. Efficient and cost effective use of resources, manpower, and technology will promote this success. We are committed to making sure that all clients have access to our resource center and receive the resources that are tailored individually to them. We believe that every patient has the right to the resources that can enhance the quality of their lives despite being faced with a chronic disease.

Health promotion Education Quality Encouragement In a safe and nurturing environment the client will be educated all aspects of end stage renal disease and about the different treatment modalities. We deliver individualized, quality care based on the bio-psychological needs of the patient. Helping the patient to acquire knowledge about the disease process, the dialysis procedure and treatment options encourages the patient to participate in his own care to the fullest extent of his capabilities and to encourages independence. We will collaborate with other health care providers and resources in meeting patient care needs.

8 Northern Michigan Dialysis Resource Center Nursing Coordinator/Manager
Reach new heights and become a part of our team Discover where compassionate care begins Seeking a Nursing Coordinator/Manager for new regional dialysis resource facility. This position serves as a mentor by role modeling a professional practice consistent with organizational goals, customer service, evidence based practice, and standards of care. NMDRC offers the opportunity to enlighten support and educate dialysis patients through this paid position. The qualified nurse manger will supports the mission statement of the Northern Michigan Dialysis Resource Center.  Plans, directs, facilitates and coordinates patient education through the effective use of assigned personnel Ensure patient rights and safety at all times. Adhere to all nursing practice standards. Lead and support quality assurance and performance improvement programs. Demonstrates knowledge, skills, and abilities related to operations and systems management, quality and customer service. Excellent communication and change management skills.

9 Making sense of dollars and cents

10 Expenses $87,000 per year for 2 full time nurses. Advertising cost of $5, per year $ 50, per year for administrative support NMDRC is committed to offering fair market wages to their full time employees. In order to attract the most qualified full time staff and volunteers, employment advertisements will require an annual budget of $5000 per year. While the goal of NMDRC is to operate through the use of an extensive network of volunteer professionals and individuals, there will be costs of $50,000 for administrative support and miscellaneous expenses

11 Funding Sources Medicare reimbursement of $200,000.00 per year
Pledged donations of 50, per year Government grant of $10, per year Our resource center is Medicare certified and any physician, nursing or psycho-social services can be reimbursed based on approved Medicare charges for dialysis patients.

12 Hours of operation: Monday thru Saturday 9:00am- 6:00pm
