A tool to help organise theories and ideas in an improvement effort Helps answer the question “What changes can we make that will result in an improvement”

What Changes Can we Make?
Primary Drivers System components which will contribute to moving the primary outcome Secondary Drivers Elements of the associated Primary Driver. They can be used to create projects or a change package that will affect the Primary Drivers.

Secondary Drivers Primary Drivers Aim Secondary drivers are elements of the associated primary driver. They contain change concepts that can be used to create projects that will affect the primary driver Primary Drivers are system components which will contribute to moving the overall aim. You may choose a particular context – e.g. dementia, older people, early years, self-directed support – to deliver this aim. By March 2016: Social services organisations understand, engage in and lead Knowledge into Practice processes, promote the benefits of Knowledge into Practice, with the necessary tools and support and a workforce deploying the relevant capabilities, to deliver those benefits. Build capacity: Build understanding, engagement and leadership among social services organisations to promote and deliver the benefits of Knowledge into Practice. Build capability: Develop social services workers’ capabilities in finding, sharing and using knowledge in day to day practice. Deploy tools and support:Embed knowledge management support, tools and techniques in service delivery and improvement in social services organisations. 7
