2 Part 1: Patient Registration

3 Upon arrival, you will be greeted by our receptionist and youll be asked to sign in to see your provider While signing in, we ask that you please provide your current address, phone number and your date of birth

4 When you visit the medical clinic, your chart is reviewed every time by a medical records clerk These clerks are responsible for making sure that we have the most accurate demographic information possible The need for updated phone numbers, addresses, insurance information, etc. is vital to providing you with the best healthcare possible.

5 After your chart is reviewed, you may be asked to complete paperwork or provide missing documents for your chart. Our receptionist may ask you to complete a new registration form or fill out a new health history. These two forms must be updated every year. We may ask you to provide a copy of your identification card, insurance card, social security card, and tribal verification if it is missing from your record.

6 You may feel overwhelmed at times by our need for updates and paperwork. I assure you that this is not nonsense The staff relies significantly on the information collected at the reception desk. For example… CHS and billing departments need this information to make educated decisions regarding payment and claims processing But, most importantly, the medical providers rely on this information to be in contact with you when it concerns your health

7 Although it is not mandated that you provide this document, it is very important that we have it. Some people may have the same name and/or same date of birth. A social security number is a very unique way of identifying our patients. Having your social security number on file can help us prevent any possible mix ups with our patients. Social Security Card

8 Tribal Verification Acceptable forms of tribal verification are: Patients BIA card, Tribal Card, Letter on Tribal Letterhead, Name on Judgment Roll or Birth Certificate along with parents tribal verification or Grandparents tribal verification, patients parents birth certificate and patients birth certificate All birth certificates must be certified (hospital birth certificates not acceptable) and they must show linkage to parents.

9 Insurance... If you have insurance, we must bill your services. Having a copy of your card on file will allow us to timely and properly bill your insurance provider. No Insurance... If you do not have insurance, we have options for you. Sliding Fee Discount We offer a sliding fee scale for those patients who qualify. You must provide proof of income, such as a recent check stub, or income tax information. Sliding Fee services must be paid at the time of your visit to receive the discount.

10 Patient Resource Advocate We also have a Patient Resource Advocate that can help you apply for resources Madonna Lopez is available to help determine which third party resource would best fit your situation. Such resources may be Medi-Cal, CMSP or Healthy Families She will assist with completing the paperwork and submitting all documentation needed to ensure that your application process is as smooth as possible Madonna also acts as an advocate for our patients with the various resource agencies.

11 Patient Resource Advocate cont… It is very important for our patients to have insurance. Having this alternate resource will allow you to become CHS eligible (if you meet all other CHS requirements) We can bill and collect from the insurance companies. LCTH can use this third party revenue to better the clinic services that we provide to our patients And, most importantly, if you become ill or injured your medical care is covered. 13 Clinic Hours Walk in clinic and appointments are available during the following hours: Monday-Thursday 7:30-3:40 Friday 7:30-1:40 Please contact the receptionist for an appointment time Ext 107

14 Walk-ins VS Appointments Walk in visits should be used for acute care only. Please be advised that you may experience a wait for you walk- in visit. Appointed patients will be seen before walk-ins. Appointments are now available all day. By having a scheduled appointment, you will be seen as close to your appointment time as possible. Appointments also allow the providers to spend more time with you discussing your health care needs.
