6 SOS Representative Responsibilities 1. Complete Hazard Identification Checklist 2. Ensure work unit members have taken the Safety Training Self-Assessment and completed EH&S required training in UC Learning Center 3. Complete work unit specific training 4. Communicate completion of steps and safety related activities to EH&S

7 Hazard Identification Checklist 1. Survey work area and identify hazards 2. Correct hazards 3. Document date corrected

8 Frequency: Hazard Identification Checklist Complete checklist every year Complete earlier if: Significant change to environment Significant change to operation New hazards Maintain SOS Records Indefinitely

9 EH&S Required Training UC Learning Center (www.uclc.uci.edu)www.uclc.uci.edu Ensure that work unit members log into UCLC and complete the Safety Training Self-Assessment Questions evaluate job categories/activities Responses determine required training Supervisors/PIs have access to transcripts

10 Work Unit Specific Activities Beyond the standard EH&S course offerings Address hazards and processes found only in your work unit Review Hazard Identification Checklist Review SOPs Review other work unit specific activities Discuss specific work unit hazards at meetings Document work unit specific training

11 Standard Operating Procedures (SOPs) Includes safe procedures and steps for hazardous work processes Examples: Emergency exit procedures Using a highly hazardous chemical Performing an operation around high voltage equipment

14 Lookout for Changes Ensure that when new hazards arise and when employees change job duties they: Retake self-assessment in UCLC Enroll in additional required EH&S classes Update Hazard Identification Checklist Receive work unit specific training

15 Rewards SOS Representative identified and has taken SOS Representative training SR receives $5 EH&S Safety Dollars SOS Representative has completed the Hazard Identification Checklist and corrected the hazards under the work units control SR receives $10 EH&S Safety Dollars Work unit members have taken Safety Training Self-Assessment and completed EH&S required training All work unit members receive $5 Starbucks gift cards All work unit members have reviewed work unit specific activities SR receives $15 EH&S Safety Dollars All work unit members receive choice of SOS items

16 Ongoing Rewards SOS Representative annually completes Hazard Identification checklist SR receives $10 EH&S Safety Dollars Work unit members participate in Rewarding Safe Acts program $5 Starbucks card for employees who are nominated.
