4 Establishing baseline for high utilization
Admission/Readmission Total Admissions 1 935 2 109 3 20 4 5 Example: EHN’s Extended Observation Unit Admissions- FY 16 Total Admissions 10% of population had at least two visits or more 2% of population had at least three visits or more High utilization is identified as 2 or 3 times in one year.

5 Establishing baseline for high utilization
Run query for all ER admissions for FY 16 Identify a service code (e.g. discharge code) that is utilized once per admission Establish a baseline for high utilization utilizing a pivot table or COUNTIF formula.

6 Using COUNTIF: After running the report, copy and paste the patient ID column into sheet2 in an excel file Remove duplicates of patient IDs in sheet2 On sheet2, in the column next to Patient IDs, select formula COUNTIF. For Range, select Patient IDs column under sheet1 and in criteria select column of patient IDs on sheet 2.
