4 Project Plan Timeline This slide shows the project plan assessment timeline for the next two months. Until December 8th, detailed reviews are being conducted. And that takes us to the 1st write back process, between December 9 and 15th. This gives us the opportunity to make any necessary adjustments or improvements to our plan. We are expecting to have this by December 9th so we can make changes and submit. Then for two weeks they will review the 1st write back responses. Then there will be a second write back process through the new year. Myers and Stauffer will submit their final findings and review to the HCA in mid-January. At this point, we will have the opportunity to drop a project if we choose. And then final project scores should be reported February 1st.

5 Project Plan Write-Back Feedback
Overall “Meets/Exceeds Criteria” on most questions No substantive changes required Care Coordination Clarify Partnering Organizations Clarify Monitoring and Continuous Improvement To review, the request for qualifications process provided us with substance needed for the project plan. These RFQs were non binding or a guarantee of funding. However, I still encourage any organizations that haven’t submitted one, to do so, because it will give us more of the information needed to hit our milestones for next year. The formal process for funding partners will be developed in the near future. The Finance Committee will develop the funding methodology for provider funding & allocation. There are still some unknowns needed to finish this process, mainly an idea of how our project plan will score and how much funding we will be awarded. I’ll go over the timeline for project plan scoring shortly.

Analyze fit between possible target populations and project start-up goals Understand existing capacity for care coordination and gaps that must be filled to implement the Pathways model Utilize input from people in the target population(s) to inform how care coordination services are designed

Sub-population data analysis (January) Compile available data Gather additional data from partners if needed Analyze and compile report Interview care coordination providers and potential CCAs (Jan. & Feb.) Meet with individuals and/or groups of partners in local communities Additional phone interviews as needed Survey or focus group process with consumers (Feb. – Apr.)
