The number of forms related to the program is indicated by the number behind the back slash sign (0/6). Step 6 – Program Application Forms 1.Complete program related Application forms. 2.Typical application forms include: Registration, Program Fees & Dates, Terms, Conditions, and Liability Waiver, Payment, and Transcript Forms. 15. Step 7 – Pay Program Application Fee Program related application fee will be invoiced after the Registration form is completed. Payments can be made by credit card, certified check or money order. Note: Application fee is non-refundable. © 2010 Toucan Education Programs
