2. The Recovery Package
The Recovery Package is a series of key interventions that together can greatly improve outcomes for people living with and beyond cancer and reduce unnecessary hospital visits/admissions.
Roll out the Recovery Package across Lincolnshire by 2020.

3. Living with and Beyond Cancer Community Project
Community project will focus on access to:
Cancer Care Reviews (GPs)
‘Navigation’ of choice following Holistic Needs Assessment
‘Health and Wellbeing Event’ of choice
Social Prescription service to meet identified needs
Services and organisations which will enable active self-management and promote health and wellbeing

4. Macmillan Community Facilitator
Lead the engagement with key stakeholders
Review existing delivery of the Recovery Package
Increase awareness of the Recovery Package with people affected by cancer
Work closely with the Macmillan Acute Care Facilitator
Initial focus on Gainsborough and Boston areas
Gain an understanding of services/groups in those areas.
Patient involvement is paramount (co-production).
Stakeholders: NHS staff, including Multi-Disciplinary Teams, patients, partners, and voluntary/third sector supporting local communities.
Existing delivery of RP: Is it fit for purpose? Identify any gaps, make recommendations.
