FSR 05 — SCAFFOLDING Understanding your Workplace Specific Critical Controls Instructions: The images in this presentation should depict the specific controls you have agreed at your workplace. Substitute these images for the actual controls you will be using and expect to see implemented.

3 Critical Control 1 ENGINEER CERTIFIED DESIGN DRAWINGS ARE AVAILABLE FOR SCAFFOLDING WITH RISK OF FALL OVER 4 METRES IN HEIGHT OR OF A COMPLEX NATURE

4 Critical Control 2 TEMPORARY WORKS ASSOCIATED WITH SCAFFOLDING IS EFFECTIVELY MANAGED

5 Critical Control 3 SCAFFOLDING IS ERECTED/DISMANTLED BY TRAINED AND COMPETENT (AND LICENSED WHERE REQUIRED) PERSONNEL

6 Critical Control 4 CHANGES IN DESIGN ARE DOCUMENTED AND APPROVED BY THE CERTIFYING ENGINEER

7 Critical Control 5 PROTECTION IN PLACE AROUND SCAFFOLDING
Exclusion zones established around scaffold structures that are close to road traffic and/or interface with plant/equipment Structural integrity is assessed prior to excavations occurring in proximity to scaffolds.

8 Critical Control 6 ALL REQUIRED GUARDRAILS ARE IN PLACE DURING ERECTION AND DISMANTLING IN ACCORDANCE WITH THE 1m LIFT REQUIREMENT * Example from Safework NSW

9 Critical Control 7 INCOMPLETE SCAFFOLD IS CONTROLLED AND ENTRY POINT PROHIBITED All access points for incomplete scaffolding must have: Signage affixed A physical barrier in place Ladders removed or covered * Image from Confined Spaces Code of Practice 2011 (NSW)

10 Critical Control 8 CONTROLS IN PLACE MANAGE DROPPED OBJECTS
Exclusion zones are in place with appropriate signage and access controls. Effective barricading (including toe boards) and/or mesh in place on scaffolding to prevent dropped objects. Where controls for dropped objects are not in place, tool lanyards are used.

11 Critical Control 9 FORMAL PERIODIC INSPECTIONS ARE CARRIED OUT BY CERTIFIED SCAFFOLDERS OR QUALIFIED PERSONS Process in place to ensure scaffolds are inspected on minimum monthly basis and after extreme weather events (Form E-T and E-T ) SCAFFTAG system (or equivalent) is in place and applied to all entry points.
