1 Restraint Forms

2 Basics All restraint forms will go to: Parents or Legal Guardian
Behavior Specialist or Behavior Analyst Education Director or Assistant Education Director Restraint Coordinator Restraint Safety Committee Department of Elementary and Secondary Education (ESE)

3 Basics All restraint forms need to be completed by the end of shift
A blank restraint form can be found on Ask Howe or as a resource on this on line training. Please fill out all restraint forms on your computer.

8 For the next two boxes “restraint start time” and “restraint end time” please remember to check off AM or PM

9 For the box marked “duration” please put how much time there were hands on for the restraint. If there were multiple restraints in one episode please put the duration of each episode separately. An example for this is: you restrained a student for 1 minute you let go and then had to retrain again for 30 seconds let go again and then had to retrain the again for 10 seconds. On the restraint form you would put 1 minute comma 30 seconds comma 10 seconds.
