1 Site Supervisor Quick Start Guide:
Psychology Practicum/Internship Application

2 Step One Definitions Account Registration
Before you can begin completing documents for the student’s practicum/internship application, you will need to set up your account. You will receive an account activation with your initial username and password from Login: Once you have logged in you may edit your username, password, and contact information my clicking on Account Information. Definitions Site Preceptor: Is used interchangeably with site supervisor. Rotation Details: Quarter in which the student plans to practicum/internship Requirements: Where you will go to complete documents to support the student’s application. Electronic Forms: Here you will see a list of required forms and status of each form. Yellow arrow : Action is needed or item is pending

3 Step Two Definitions Completing & submitting required documents
There are a few portions of the application that we ask site supervisors to complete. All application materials are due by a set deadline, your student should inform you of this date. Click the My Requirements tab on the left navigation panel to see a list of all of the required documents. You will see that some are required and other optional. Be sure to review all optional items to determine they are needed in your situation. To the far right of each document title you will see an Edit button, click on this to start each document. If the requirement is to complete a form, click on ‘Open New Form’ or select form from a dropdown list if provided. If the requirement is to upload a document such as a resume or proof of insurance, click on ‘Choose File’ and locate the file to upload. Once the requirement is complete, enter the date in the Completed On field and select ‘Complete’. If the requirement is still in process select ‘In-Process’. Be sure to click on save changes at the bottom of the form if you need to complete it at a later time. Definitions Requirements: where you will go to work on required application documents. 4 Step Three: After submission
Definitions Ready for Practicum: this submitted once you have fully completed all other requirements & are ready to submit your application for review. Electronic Forms: here you will see a list of required forms and status of each form. Requirements: where you will go to work on required application documents. Rotation Details: Quarter in which you will begin practicum/internship Site Preceptor: used interchangeably w/ site supervisor Step Three: After submission You will be contacted via by the Psychology Clinical Training Department if there are any questions or revisions needed on the documents you submitted. The initial review process takes approximately three weeks. Once all portions of the students application materials are received and determined to be in good format, you will receive an from the Psychology Department’s Clinical Training Department requesting you to review their documents. This step ensures that the student has accurately reflected both the site and the training experience in their application. Yellow arrow : action is needed or item is pending
