© The Risk Management Tool Box January 2011 Hazard Spotting Test – Incorrect Answers None of these things are “hazards” You’d be wrong if you said “no handrails” on the scaffold. You’d be wrong if you said “inadequate flooring” (i.e., holes) on the scaffold. The image used here is ©Safetyphoto You’d be wrong if you said “ladder used for purposes other than access or egress “(i.e., ladder used to carry-up render). You’d be wrong if you said “no toe-boards” on the scaffold. You’d be wrong if you said “untidy job” or “poor housekeeping”. You’d be wrong if you said “lack of access control or “no barricading” of the work site. You’d be wrong if you said “failure to secure the scaffold” (i.e., freestanding tower). You’d be wrong if you said “lack of use of appropriate PPE”. Other incorrect answers include: Inadequate supervision; Inadequate contractor selection/management; Overloading scaffold; Ladder too short for access to top level; Inclement weather (e.g., windy day); or Unsecured tools and objects at height. You’d be wrong if you said “scaffold not built to acceptable standard”. 1

Simple – like 99% of workers – you can’t identify hazards because you’ve never been trained to correctly understand what you’re looking for! There are now three things you can do: Do nothing and continue to fail to identify hazards; Work it out for yourself; Contact a HSE Professional in you location who understands the energy model of hazards.
