2 What are triggers? Triggers are the mechanisms that make hazards go out of control. If you think about this, it makes sense. Hazards have the potential to cause harm. But so long as we’re managing them, we’re perfectly safe. For an accident to occur, there has to be something that causes the hazard to go out of control. That something is a triggering mechanism! Triggers release the hazard and make them go out of control.

5 Correct answers All of these things are “triggers”
Untidy workplace (poor housekeeping). All of these things are “triggers” Uncontrolled sources of ignition present (i.e., power generation). Not gas testing or monitoring. Lack of use of appropriate PPE, no hard hat, no respirator, no chemical resistant clothing, etc. Lack of access control (no barricading of the work site). No compliance to standard Permit to Work requirements, no fire watch, no sentry, no register, no retrieval system.
