Performing Annual Screening Mammograms
Open to Ladies 40 years old or older (no order needed);
Ladies years old will need a physician’s order;
Must provide physician contact information;
Must be able to climb 5 steps;
Bring your insurance card; and
Photo Identification (driver’s license/passport)
Please bring your prior mammography films
or the name of the facility where they were done.
