2. Before you can get credit…
Your hospital needs to be finished with the QI project and be certified as “ready” to recruit patients
You need to have participated in all required QI activities for the project, including:
Attending initial training and unit-based team QI meetings (minimum of 4 meetings)
Reviewing practice audit data from PDSA improvement cycles

3. Log in to the ABP website: www.abp.org

4. Click on “My MOC Requirements”

5. Click on “Search Activities”

6. Click on “Search Part 4”

7. Under “Sponsor,” Select “Children’s National Health Network & DCPICHQ”

8. Click on “Search Approved Activities”

9. You will click on either “Breastfeeding: A Nursery QI Project” or “Safe Sleep: A Nursery QI Project”
Scroll down to find “Safe Sleep: A Nursery QI Project”

10. Make sure that you have completed Completion Criteria
Make sure that you have completed Completion Criteria. Click on “Attestation Form”

11. Complete Attestation Form
Most of the fields will be pre-populated
Complete all of the blanks
You need to check all of the criteria listed in Q7 to receive MOC credit
Print out the form
Sign the form at the bottom (not shown)
Submit the form to Rachel Moon at
You should see documentation of your MOC credit on the ABP website within 1 month
