(A) An incision of 3 cm is made in the ventral aspect of a presumed thoracotomy line and the operator’s index finger is introduced into the chest cavity to assess the duration of the empyema and to free the underlying lung. (B) A Senning suction device (Ulrich AG, St Gallen, Switzerland) is introduced. (C) Fibrin and loculations are evacuated under endoscopic vision.
